2. Provide information requested for each courtesy vehicle to be permitted on the attachment for which a permit decal and barcode is requested. Attach a copy of the current registration and leasing agreement, if applicable, for each vehicle. (See the Ground Transportation Rules and Regulations for definition of “Class 1, 2, 3” vehicle). Attach additional sheets, as required. If the vehicle(s) to be used are already included in the primary application, it is not necessary to list them again. This information should also be provided, in writing, each time a vehicle is added to or deleted from the fleet.

4. NOTE: Courtesy vehicles must use Access Control Media to enter Commercial Lane. Any Access Control Media provided by the Authority remains the property of the Authority, and is subject to confiscation in accordance with established procedures. Replacement of any Access Control Media that is damaged, mutilated or lost may result in a fee set by the Authority. The company may be held liable for fees incurred using lost/stolen Access Control Media. No cash transactions may take place on Commercial Lane. NO EXCEPTIONS.

5. Complete the surety requirement using the bond or letter of credit formats provided. Surety instruments should be in place as soon as possible. New surety amounts are enclosed.

6. Provide the Authority monthly gross receipts reports and pay appropriate privilege fees
in accordance with the rules and regulations.

7. The above named company, its representative and employees agree to operate within the guidelines of the Ground Transportation Rules and Regulations for the Landside Terminal Building at Orlando International Airport, Orlando, Florida, as established and adopted by the Greater Orlando Aviation Authority on June 18, 1986 or as thereafter amended and the concession agreement (if applicable) between the Company and the Authority. I certify that all information provided on this application is true and correct.
